Reporting hazards and near misses

A near miss is an event that could have caused injury or damage but didn't. Reporting near misses is one of the most effective ways to prevent the next real incident.

When you report, include:

  • What happened, where and when.
  • What could have happened.
  • Anything you did to make the area safe.
